Risk for Injury related to photophobia, pseudoptosis

Expected outcomes:
  • The injury did not occur.
  • Identifying factors that increase the risk of injury.
  • Expressed a desire to take security precautions to prevent injury.

Intreventions:

1. Limit activities such as moving the head suddenly, scratching the eye, bowing.
Rational: reducing the risk of falls or injury.

2. Orient the patient to hold the tools needed environment to the patient's body.
Rationale: prevent injury, increase independence.

3. Set the environment around the patient, keep objects that can cause accidents.
Rationale: to minimize the risk of injury, provide comfort for the patient.

4. Keep an eye on or accompany the patient while doing the activity.
Rationale: control the activities of the patient and decrease safety hazards.

5. Clean the eye secretions in the right way.
Rationale: eye secretions will make blurry vision.

6. Note grievances blurred vision that can occur after the use of eye drops and eye ointments.
Rationale: Provides information on the client not to perform malicious activities immediately after use eye medicine.

7. Use sunglasses.
Rationale: Reduces photophobia that can impair vision on the patient.
