Impaired Skin Integrity related to neuromuscular damage (pain)
Purpose: The patient can improve the mobilization at the highest level.
NOC: Mobility level
Expected outcomes:
- Balance appearance.
- Positioning the body.
- Muscle movement.
- Joint motion.
- Walk ambulation.
- Wheelchair ambulation.
NIC: Exercise Therapy: Ambulation
- Help the patient to use the facilities of walking aids and prevent accidents or falls.
- Place the bed in a position that is easy to reach / achieve the patient.
- Consult a physiotherapist about the plan ambulation as needed.
- Monitor the patient in the use of walking aids others.
- Instruct the patient / ambulatory care providers about ambulation techniques.
