GCS (GLASGOW COMA SCALE) is a scale that is used to determine or assess the patient's level of consciousness, ranging from a fully conscious state to a state of coma.
On examination of consciousness or GCS, there are 3 functions (E, V, M) to be examined, each function has different values, for the following explanation.
E: Eyes / Eye: total value of 4
V: Verbal: total value of 5
M: Motor / motion: the total value of 6

HOW TO ASSESSMENT
Type of Examination | Value | Response |
Eye Response Spontaneous Voice stimuli Pain stimuli Nothing | 4 3 2 1 | Eyes open spontaneously Eyes open to verbal commands Eyes open to pain stimuli Do not open our eyes to any stimulation |
Verbal Response Good orientation Confused Say the words that are not appropriate Say the words are not clear Nothing | 5 4 3 2 1 | Good orientation and is able to speak Disoriented and Confused Repeating words improper randomly Moan or groan No response |
Motor Response Obeying orders Localize stimuli Pulling stimuli Abnormal flexion Abnormal extension Nothing | 6 5 4 3 2 1 | Can move to follow orders Can localize pain (directional movement and aiming in the direction of painful stimuli) Flexion or interesting moment in pain stimuli example: draw a hand when the nail on tap Forming decortication position. Example: wrist flexion Forming decerebrate position. Example: wrist extension No response, just lay weak, while in any excitatory |
